Players Minimum required to confirm a team's registration. Additional players can be added at a later date.

*Please include Your:

  1. Team Name

  2. Division

  3. Region

  4. Team Manager Name

  5. Player Names (4 players minimum required to confirm a team's registration)

  6. Player Skill Ratings

A PayPal payment request will be sent once your team registration is received. Teams are not confirmed until payment is received.
